4D Health 1 (€1045.40 per adult, €276.40 per child) provides cover for a semi-private room in private hospitals with a €200 inpatient excess per claim and a €2,000 co-payment on certain orthopaedic procedures. It also provides cover for a semi-private room in the Beacon Hospital with a €200 inpatient excess per claim. Day Case cover is provided in private and high tech hospitals with a €75 excess per claim. 3 nights cover in a private room in public hospitals is provided for maternity accommodation.

 

4D Health 2 (€1271.90 per adult, €313.80 per child) provides cover for a semi-private room in private hospitals with a €150 inpatient excess per claim and a €2,000 co-payment on certain orthopaedic procedures. It also provides cover for a semi-private room in the Beacon Hospital with a €200 inpatient excess per claim. Day Case cover is provided in private and high tech hospitals with a €75 excess per claim. 3 nights cover in a private room in public hospitals is provided for maternity accommodation.

 

4D Health 3 (€1441.70 per adult, €407.80 per child) provides cover for a private room in private hospitals with a €75 inpatient excess per claim and a €2,000 co-payment on certain orthopaedic procedures. It also provides cover for a private room in the Beacon Hospital with a €75 inpatient excess per claim and full cover for listed cardiac procedures in High Tech Hospitals with a €75 excess per claim. Day Case cover is provided in private and high tech hospitals with a €50 excess per claim. 3 nights cover in a private room in public hospitals is provided for maternity accommodation. This plan also includes outpatient benefits such as  50% back on GP and Consultant visits and 50% back on dentist visits up to €300 per policy year.

 

4D Health 5 (€1668.20 per adult, €453.30 per child) provides cover for a private room in private hospitals with a €50 inpatient excess per claim and a €2,000 co-payment on certain orthopaedic procedures. It also provides cover for a private room in the Beacon Hospital with a €50 inpatient excess per claim and full cover for listed cardiac procedures in High Tech Hospitals with no excess. Day Case cover is provided in private and high tech hospitals with no excess. 3 nights cover in a private room in public hospitals is provided for maternity accommodation. This plan also includes significant outpatient benefits such as 75% back on GP and Consultant visits and 50% back on dentist visits up to €300 per policy year.
